The GE Foundation has gifted $3 million under the company’s Developing Health program to Health Choice Network of Florida (HCNFL). The endowment will help start a Care Management Medical Home Center for 10,000 Miami Dade chronic diabetes patients. Developing Health is a $50 million program that seeks to improve primary care accessibility in identified communities throughout the country.

The grant will enable HCNFL and its seven participating health centers to offer a centralized model with medical professionals helping the health center teams provide efficient care management services. It will also help in the setting up of real-time disease-specific patient panels and identification of health trends. GE Foundation is the philanthropic arm of General Electric Co.
